Reentry Navigation & Case Management

Personalized support to help participants navigate reentry systems, enroll in eligible benefits, and connect with housing, employment, health care, and community-based resources. This service is intended as an initial support to help reduce immediate barriers during reentry.

Basic Needs & Stabilization Support

One-time assistance designed to meet immediate needs during reentry. Services may include Coming Home Resource Kits, limited gift card support for essentials, hygiene supplies, and access to feminine care products to support health, dignity, and stability.
